I. JOB IDENTIFICATION

A. TITLE: Social Case Worker

B. REPORTS TO: Director of Vision Rehabilitation Services

C. FLSA STATUS: Exempt

II. STATEMENT OF PURPOSE

The Social Worker is responsible for the evaluation of individuals to maximize self-sufficiency and independence in the home and/or work environment. Proficient teaching, problem solving, cooperating, and communication with internal/external service providers are key elements of the position, to ensure quality of service and consumer satisfaction. Social Worker works in a strong team environment.

III. RESPONSIBILITY FOR THE WORK OF OTHERS

May supervise and train student interns.

IV. BUSINESS COMMUNICATION

A. Communication with the Vision Rehabilitation Services Director is frequent.

B. Communication with staff, i.e.: Vision Rehabilitation Therapist, Orientation and Mobility Instructor is ongoing.

C. Communication with eye doctors, NYSCB counselors and other vision related professionals is regular.

D. Communication with human service agencies is occasional.

E. Communication with visually impaired consumers as well as their family members and significant others when appropriate is ongoing.

V. SUPERVISION REQUIRED

This position requires the ability to work independently and in close coordination with the Vision Rehabilitation Services Manager.

VI. EDUCATION, TRAINING AND SPECIALIZED KNOWLEDGE REQUIRED

This position requires a Bachelor’s in Social Work (BSW), or a bachelor’s or master’s degree in a related social/human services field with a minimum of one year of work social or related experience. 

VII. SPECIFIC DUTIES AND RESPONSIBILITIES

A. Conduct home visits with individuals who are legally blind to complete intake assessments and determine vision rehabilitation needs for the Adaptive Living Program cases.

B. Enter assessment findings into NYS Commission for the Blind portal to develop a plan.

C. Manage Adaptive Living Program cases including communication with appropriate AVRE staff to initiate direct service provision of Vision Rehabilitation Therapy and Orientation & Mobility Instruction based on intake assessment.

D. Receive case notes from Vision Rehabilitation Therapists and Orientation & Mobility Instructors to complete the final reporting for the NYS Commission for the Blind.

E. Provide Social Casework services to consumers including adjustment to blindness and access to community resources for a variety of social issues.

F. Complete case notes and reports required by the Association, contract and referral source requirements.

G. Collaborate with staff to ensure continuity of services.

H. Keeps workplace organized and clean.

I. Perform other duties as assigned.

J. Maintains strict confidentiality in all aspects of AVRE activities.
